The first step to using community resources to address patients’ social and behavioral health needs is to identify community agencies and other organizations that can help meet those needs. Many hospitals perceive that there are limited or no community resources available, without having made a concerted effort to look for these resources.
A community asset (or community resource, a very similar term) is anything that can be used to improve the quality of community life. And this means: It can be a person -- Residents can be empowered to realize and use their abilities to build and transform the community.

Typically, when someone refers to a community resource, they mean an organization that serves a particular geographical area or group of people by providing tools to help that community grow in positive ways and improve the quality of life for the people of that community. As such, community resources can be run or funded by the government, businesses, non-profit groups, or even individuals and serve the community in a variety of ways.
Early care and education (ECE) professionals who learn about their community’s resources, and connect families with these resources, provide an important bridge to services for children and families. Community-based services include a variety of supports and services for children and families. Services may focus on children’s developmental needs, for example, early intervention services like screening children for speech and language delays, or physical delays.

Student volunteer models for social needs screening, referral, and follow-up have been developed and implemented across the country, but evaluation is sparse and translation to federally qualified health centers—where patient need is greatest—is lacking.
Question: Can hospitals report as community benefit programs that connect patients to resources in the community, like Health Leads? As more and more organizations focus on the social determinants of health, we are identifying workforce capacity to help patients connect to affordable housing, food security, and employment.
People's incentives to follow and participate in a community resource management plan will be stronger when they feel that their interests and concerns are represented in that plan. The importance of including stakeholders in the decision-making process will be addressed further in Chapter 5.
